I've seen this happen (not in the mouth though). Pneumatic nail guns make a loud pop sound and recoil on each shot. People would get shot in the arm by a misfire, have a nail 3 inches into their forearm, and not notice it for hours. Wasn't uncommon at all.

I guess the shock of the recoil masks the impact of the nail, and the speed that the nail enters makes the pain relatively minimal and the wound small/tight enough not to bleed around the nail.

Into the mouth/brain.. I dunno. Maybe the fact that he had a nail sticking in his brain and it didn't have any noticeable effect says a lot.

I could understand how he might get distracted from the pain of the nail. There aren't that many nerves in the roof of your mouth, and very few/none in your skull/brain. I'm more concerned about how it managed to get in there at that particular angle...
